Why Waist Circumference Matters for Metabolic Health

Kevin Aniel

Body weight alone does not tell you where body fat is stored.

This article is part of the AKK Knowledge Centre, our guide hub for metabolic wellness, gut health and Akkermansia research.

That matters because fat stored around the abdominal organs — visceral fat — is more strongly associated with cardiometabolic risk than fat stored under the skin in many other areas.

Waist circumference is a simple screening tool used to estimate abdominal obesity risk.

It is not a direct measurement of visceral fat.

But it can provide information that BMI alone misses.

For Asian adults, Singapore HealthHub uses lower waist thresholds than many Western references:

  • approximately 90 cm or more for men
  • approximately 80 cm or more for women

indicates increased abdominal-obesity risk.

These are screening cut-offs, not a diagnosis by themselves.

Key Takeaways

  • BMI and waist circumference measure different things.
  • BMI estimates weight relative to height.
  • Waist circumference estimates abdominal-fat risk.
  • Visceral fat is biologically active and is associated with insulin resistance and cardiovascular risk.
  • A person can have a normal BMI and excess abdominal fat.
  • Asian populations can develop metabolic risk at lower BMI and waist values.
  • Waist circumference should be interpreted with blood pressure, glucose, lipids and other factors.
  • The goal is reducing health risk, not achieving one cosmetic waist size.

What Is BMI?

Body mass index is calculated as:

weight in kilograms ÷ height in metres²

BMI is useful because it is:

  • inexpensive,
  • simple,
  • standardised.

At a population level, higher BMI is associated with greater risk of many chronic diseases.

But BMI has limitations.

What BMI Cannot Tell You

BMI does not directly measure:

  • body-fat percentage,
  • muscle mass,
  • visceral fat,
  • fat distribution,
  • fitness.

A muscular athlete can have a high BMI without high body fat.

Another person can have a relatively normal BMI but high abdominal fat and low muscle mass.

What Is Waist Circumference?

Waist circumference measures the size of the abdomen using a tape measure.

It acts as an indirect marker of central or abdominal adiposity.

Singapore HealthHub recommends measuring around the abdomen near the midpoint between the lower ribs and top of the hip bone, typically during exhalation.

Consistency matters more than measuring at random locations.

What Is Visceral Fat?

Visceral fat lies around internal organs.

It differs from subcutaneous fat, which lies under the skin.

Visceral adipose tissue is metabolically active.

Higher visceral fat is associated with:

  • increased fatty-acid release,
  • liver fat accumulation,
  • insulin resistance,
  • altered inflammatory signalling,
  • dyslipidaemia.

This is why abdominal fat matters beyond appearance.

Can You Have High Visceral Fat With a Normal BMI?

Yes.

This is sometimes informally described as:

“thin outside, fat inside.”

A person may have a body weight in the normal range while carrying excess fat around the organs.

This can happen especially with:

  • low muscle mass,
  • physical inactivity,
  • ageing,
  • genetic predisposition.

Why Are Asian Cut-Offs Lower?

Asian populations can develop type 2 diabetes and cardiovascular risk at lower BMI and waist values than some European-origin populations.

This is why Singapore uses Asian-specific risk thresholds.

HealthHub identifies abdominal obesity risk around:

  • ≥90 cm for Asian men
  • ≥80 cm for Asian women

This does not mean everyone crossing the threshold has metabolic disease.

It means risk is higher and further assessment can be useful.

What About BMI in Singapore?

Singapore also uses lower BMI risk categories for Asian adults than conventional global WHO categories.

HealthHub commonly classifies:

  • 18.5–22.9 kg/m² as a lower-risk healthy range,
  • 23.0–27.4 kg/m² as increased/moderate risk,
  • ≥27.5 kg/m² as high risk for weight-related health problems.

BMI is still only one screening tool.

Waist Circumference and Metabolic Syndrome

Increased waist circumference is one of the major risk components included in metabolic syndrome frameworks.

Other components include:

  • elevated glucose,
  • elevated triglycerides,
  • low HDL cholesterol,
  • elevated blood pressure.

Waist circumference is therefore most useful as part of a cluster.

Is Waist-to-Hip Ratio Better?

Waist-to-hip ratio compares waist circumference with hip circumference.

It can add information about fat distribution.

But it requires two measurements and can introduce more measurement error.

Waist circumference alone remains simple and widely used.

Does Losing Waist Size Improve Metabolic Health?

Often, reductions in abdominal fat accompany improvements in:

  • insulin sensitivity,
  • blood pressure,
  • triglycerides,
  • liver fat.

But the health goal should not be:

“smallest waist possible.”

It should be reducing excess abdominal adiposity when it contributes to risk.

Can Exercise Reduce Visceral Fat Without Large Weight Loss?

Yes.

Exercise can reduce visceral fat even when changes on the scale are modest.

Aerobic exercise is especially well studied.

Resistance training helps preserve or increase muscle mass.

Combining movement and dietary change is often effective.

What Kind of Diet Helps?

No one food specifically removes belly fat.

A sustained dietary pattern that helps manage total energy intake and diet quality can reduce abdominal fat over time.

Useful principles include:

  • adequate fibre,
  • minimally processed foods,
  • appropriate protein,
  • portion awareness,
  • fewer sugar-sweetened drinks,
  • limited excessive alcohol.

Does Alcohol Affect Abdominal Fat?

High alcohol intake adds energy and can influence liver and fat metabolism.

Reducing excessive alcohol can support weight and metabolic management.

But “beer belly” is not a special biological category.

Overall energy balance and individual fat distribution matter.

Can You Spot-Reduce Belly Fat?

No.

Abdominal exercises strengthen abdominal muscles.

They do not selectively remove visceral or subcutaneous fat from the waist.

Fat loss occurs through whole-body energy balance and metabolic adaptation.

How Often Should You Measure Your Waist?

HealthHub includes waist circumference as part of general adult weight-risk screening.

For personal tracking, measuring occasionally under consistent conditions is enough.

Daily waist measurements are unnecessary because:

  • hydration,
  • food,
  • bowel contents,
  • measurement position

can create noise.

Should You Track BMI or Waist?

Use both when relevant.

BMI provides a broad weight-for-height measure.

Waist adds information about fat distribution.

Combine them with:

  • blood pressure,
  • glucose,
  • lipids,
  • physical fitness,
  • family history.

Frequently Asked Questions

What waist circumference is considered high for Asians?

Singapore HealthHub uses approximately 90 cm for men and 80 cm for women as abdominal-obesity risk cut-offs.

Is waist circumference better than BMI?

Not simply better — complementary.

Can skinny people have visceral fat?

Yes.

Does belly fat cause insulin resistance?

Higher visceral fat is strongly associated with insulin resistance and can contribute through several biological pathways.

Can sit-ups reduce visceral fat?

No. Spot reduction is not supported.

The Bottom Line

Waist circumference matters because where fat is stored can be as important as total body weight.

BMI remains useful.

But adding waist circumference can reveal abdominal risk that BMI may miss.

For metabolic wellness, use these tools as screening information — not as cosmetic targets or standalone diagnoses.
